At AKDHC, physicians

extends from our offices into the

physicians work with a multidisciplinary
team in
the evaluation, management and
education of patients with chronic
kidney disease. Our goal

of CKD.

the community, AKDHC physicians participate
in screening programs,
education programs
and collaborate with
primary care providers
in early identification
and management of individuals
with CKD.
