Medical Records

Your original medical record is the property of AKDHC, but the information in it belongs to you.  You have the right to request copies of your health records and are able to view and print pertinent records through our online Patient Portal.  

Options for Accessing Your Health Information:

Patients who register for the AKDHC Patient Portal have access to pertinent health information 24/7 without having to complete a request or authorization form.  Visit summaries will be available for printing and viewing and secure messages can be sent directly to your physician’s staff.  Create a portal account at AKDHC Patient Portal to begin accessing your Health Information through our convenient and easy to use Patient Portal. 

1. 2. To request copies of other health information please complete the Patient Request for Medical Records form and mail the completed form to the address identified on the form.  Completed forms can also be given to office staff and they will direct the form to the appropriate personnel responsible for completing your request. 

Typical processing time for copies of records is 7-14 business days.  Delays in processing may occur if the request form is incomplete.
